Making an Appointment
To make an appointment at Valley Mental Health Tooele Unit, call our office at 843-3520, or stop by 100 South 1000 West, Tooele. Our support staff will talk with you briefly, asking you a few questions to determine what therapist will best fit your needs. You will also be asked some questions to help us gather demographic and insurance information.
Assessment
You will be scheduled to come in for your first appointment 30 minutes prior to appointment time with your therapist to complete the required paperwork. During your assessment, the therapist will ask you detailed information regarding your situation, concerns, and changes you would like to make. After the assessment session, the therapist will make recommendations for your treatment needs, and together, you and your therapist will develop a treatment plan to help improve your situation or concerns. Based on your assessment, you may be assigned to another therapist who can better meet your needs.
Treatment Options
Treatment options at the Tooele Unit consist of the following: individual therapy, couple therapy, family therapy, group therapy, or medication evaluation and management.
Crisis
Valley Mental Health Tooele Unit provides 24 hour crisis services for Tooele County. If you are in need of a crisis worker Monday through Friday (8:00 a.m. - 5:00 p.m.), please call our office at 843-3520, and request to speak with a crisis worker. If the crisis is after normal business hours or on the weekend, please call 882-5600 and request a crisis worker to contact you. The crisis worker will handle the immediate crisis and refer you for other services as needed.
Fees
Our fees are based on a state approved sliding fee scale (with the exception of court ordered clients or Medicaid clients). You will be expected to make your individual fee payment at the time of service.
Insurance
Your information will be gathered and your insurance will be billed. Please assist us by bringing your insurance cards with you at the time of your first appointment. Also, you will be expected to supply any additional information to your insurance that they require prior to their making payment to us. Be aware, most insurance companies pay 50% of the full cost of services for mental health and substance abuse treatment. You will be expected to pay your individual assessed fee unless your insurance covers 100%.
Court Ordered
Those individuals referred in for court ordered assessments will be charged a flat assessment fee, regardless of income (unless you are on Medicaid), and your insurance will be billed. Fees for individual treatment will be based on your income. If you are referred to group therapy, there is a flat group fee. You will be expected to bring your assessment fee to the first appointment. You will also be expected to make your individual or group fee payment at the time of service.
Canceling Appointments
Please call at least 24 hours in advance to cancel your appointment, so that we may use that time for someone who is on a waiting list. If you are more than 15 minutes late (or 5 minutes late for the psychiatrist), we will need to reschedule your appointment.
Confidentiality
Confidentiality of your records is of utmost concern, because of both legal requirements and our ethical commitment to you. We cannot and will not communicate information regarding you with anyone, unless you sign a “Release of Confidentiality” form, giving us permission to do so. This is helpful when working with other agencies (school, doctor, legal) to assist in your treatment. There are some legal exceptions to your confidentiality: 1) if you disclose intent to harm yourself or others, 2) if there is child or elder abuse or neglect occurring, or 3) if a court subpoenas your record.
Making Referrals - Information for our community partners
School counselors and administration, physicians, attorneys, DCFS workers, probation officers, judges, and others often refer people for our services. Here are some guidelines to help you in making the referral.
We get clients in for services as soon as possible, and will not normally be able to get them in the same day that a referral is made.
