Case Management Team

Valley Mental Health Tooele Unit Case Management Program is a care delivery system for clients diagnosed with a severe and persistent mental illness. Clients who require extensive outpatient services to promote independence and recovery in the community may be referred to the case management program by their primary therapist. Case management support is provided to assist clients in effectively utilizing available community resources. This service can also assist clients in accessing government resources. The case management unit utilizes all available resources within the unit and community to strengthen the quality of our clients personal, family, and community life.

Representative Payee Program

As part of the case management unit, the representative payee is available. The representative payee program is a program to assist individuals diagnosed with a serious and persistent mental illness manage their finances. Clients and their representative payee work together to budget the client’s money to ensure that shelter and food needs are met. Representative payees assist clients who voluntarily request payee services to help with money management, or those who are mandated by Social Security to obtain a payee to manage their benefits. The payee also plays an active role in advocating for the client in financial crisis, and coordinating with Social Security and other agencies that require reporting of the client’s income. Clients meet with their payee at least once a month to discuss upcoming bills and expenses. The Representative Payee and client then develop a monthly budget to include basis needs such as food and rent. The representative who works in conjunction with our outpatient unit, case manager, primary therapist, and psychiatrist to accommodate their clients who are in need of payee services. Representative payees are also responsible for reporting any changes in the client’s income or living situation to Social Security and the Department of Workforce Services. Referrals to the Tooele Representative Payee Program are to be made through the client’s primary therapist. When a client is assigned a representative payee, it usually takes 6-8 weeks from the date of application to start receiving benefits on the client’s behalf.
