Assessment Treatment Termination In case of crisis please call 1-800-537-8739. The crisis worker will manage the immediate crisis and contact your therapist as soon as possible to inform him/her of the situation. Fees Fees are paid typically paid either by one's own insurance, medicaid, or as based by one's ability to pay as determined by a sliding fee scale. No one is refused services due to financial concerns. Confidentiality Your confidentiality is our utmost concern both out of legal requirements as well as our ethical commitment to you. This means that we can not and will communicate your personal information with anyone unless you give us permission by signing a Release of Confidential Information Form. This is helpful at times when working with others (school, legal system, doctors, etc) to aid in your treatment. Otherwise your information is strictly confidential. It is important to note that there are some legal exceptions to your confidentiality: 1. If you disclose intent to harm yourself or another; 2. If there is child or elder abuse or neglect occurring; or 3. If a court subpoenas your record. Canceling Appointments We ask the courtesy of canceling appointments at least 24 hours in advance so that time may be made available for another person who may need it. Appointments not canceled 24 hours in advance will be charged a late cancellation fee. If for some reason you need to be more than 20 minutes late for your appointment we will need to reschedule your appointment.
