Intake
  • Contact our office at 435-649-8347 or stop by our office at 1753 Sidewinder Dr., Suite 200, Park City, UT.
  • Support Staff or a Therapist will talk with you briefly to gather contact information and a description of your concerns. This is called the Intake Interview.
  • That person will then give your intake information to the Program Manager assigns a therapist to conduct a more thorough Assessment of your needs.
  • Therapists are assigned based on therapists expertise to meet your needs, office location, scheduling considerations and other factors.
  • Typically, you will be contacted within 24 hours of your Intake Interview to schedule your first session with your therapist, otherwise known as the Assessment Session.

Assessment

  • Be prepared to come 15-20 minutes early to your first session to fill out paperwork.
  • During the Assessment Session your therapist will ask you for more detailed information concerning your situation, concerns, and changes you would like to make. Feel free to ask your therapist any questions you may have.
  • After the Assessment Session your therapist will make recommendations for treatment based on your needs. Clients and therapists develop treatment plans together.

Treatment

  • Treatment typically consists of a one or more of the following: individual therapy, couple therapy, family therapy, group therapy, medication evaluation and management, educational classes.
  • Group therapy is a key component for most people's treatment as it is one of the most effective forms of treatment. Because of this your therapist will recommend a group that fits your needs.
  • Be sure to ask your therapist any questions you have about the methods he/she is using and also to express any of your concerns about your treatment.
  • You may periodically be mailed surveys asking for your feedback on your treatment experience.

Termination

  • Termination (closing) of your case occurs for different reasons: 1. When you and your therapist agree that your goals for treatment have been met and you no longer require our services; 2. You have not made contact with us in over 90 days; 3. You have been referred to another therapist or agency; or 4. for other factors.
  • Closing your case is simply a standard procedure we follow. You are always welcome to contact us again if you ever need further services and we will be happy to re-open your case.
Crisis

In case of crisis please call 1-800-537-8739.

The crisis worker will manage the immediate crisis and contact your therapist as soon as possible to inform him/her of the situation.

Fees

Fees are paid typically paid either by one's own insurance, medicaid, or as based by one's ability to pay as determined by a sliding fee scale.

No one is refused services due to financial concerns.

Confidentiality

Your confidentiality is our utmost concern both out of legal requirements as well as our ethical commitment to you. This means that we can not and will communicate your personal information with anyone unless you give us permission by signing a Release of Confidential Information Form. This is helpful at times when working with others (school, legal system, doctors, etc) to aid in your treatment. Otherwise your information is strictly confidential. It is important to note that there are some legal exceptions to your confidentiality: 1. If you disclose intent to harm yourself or another; 2. If there is child or elder abuse or neglect occurring; or 3. If a court subpoenas your record.

Canceling Appointments

We ask the courtesy of canceling appointments at least 24 hours in advance so that time may be made available for another person who may need it. Appointments not canceled 24 hours in advance will be charged a late cancellation fee.

If for some reason you need to be more than 20 minutes late for your appointment we will need to reschedule your appointment.

School Counselors, Physicians, Attorneys, DCFS Workers, Probation Officers, Judges, Therapists, and others often refer people for our services. Here are some guidelines to help you in making your referral.
  • Encourage your client to contact our office at 435-649-8347.
  • For us to be able to communicate with you by phone or correspondence regarding your client be sure to: 1. let us know of your desire to do so as when a client comes to see us they don't always inform us that you have referred them to us; 2. Have your client sign a release of confidential information form allowing us to communicate with you.
  • You will be periodically surveyed regarding your experiences of referring your clients to us and working with us.
  • Be sure to ask us any questions you may have about the nature and process of treatment, referrals, etc.
