Procedures for Psychology Internship Application

The following steps must be completed in order to have your application for an internship approved for review:

1. Read and follow the instructions for the APPIC Application for Psychology Internship (AAPI) found at www.appic.org;
    
2. Complete Parts I and II of the APPIC Application for Psychology Internship (AAPI) found at www.appic.org;
    
3. Send a copy of your vitae as well as official transcripts from your graduate school;
    
4. Send three letters of recommendation and the accompanying Valley Mental Health Internship Program Evaluation Form along with the APPIC Application to 1141 East 3900 South, Suite A-200, Salt Lake City, UT  84124.  One of these letters and Evaluation Forms must be completed by your training director.  The Evaluation Form can be found on our website at www.vmh.com (click on Psychology Internship Application);
    
5. All documents must be mailed to Liz McGill, Ph.D. at Valley Mental Health and postmarked by the November 30, 2008 deadline. If you have any questions concerning the application, please feel free to contact Dr. McGill by e-mail or at 801-264-2300; and
    
6. You may download and complete the Psychology Internship Application from the internet and e-mail it to lizm@vmh.com
