Advance Directives: An Advance Health Care Directive is a form that helps others give you the care you would want when you cannot make decisions.  Other names for the form are “Special Power of Attorney for Health Care", “Durable Power of Attorney for Health Care,” or “Living Will.”

Medicaid Member Handbook

Summit and Tooele Counties: Medicaid enrollees living in Summit or Tooele County has Valley Mental Health as their mental health provider. This handbook explains the Medicaid mental health and substance abuse services that Valley Mental Health covers.
